The Special Populations Health Workgroup addresses continuum of care issues, accessible hours of operation/locations, collaborative relationships, quality improvement/assurance, service level program expectations, and quality of care, health outcomes/disparities, and  performance measures specific to Migrant Health Center, Health Care for the Homeless, and Public Housing Primary Care grantees. Participants are encouraged to share concerns and feedback at all meetings as well as to provide specific topics for meetings. The group shares promising practices, discusses challenges, exchanges resources, and partners on initiatives to facilitate the delivery of quality, comprehensive, and culturally appropriate health care services to patients of health centers receiving special populations funding. This workgroup is staffed by NCCHCA’s Community Development & Special Populations Coordinator with the assistance of the Director of Training & Technical Assistance.
